      Medical record files will be destroyed for patients who have not received services for more than 5 years or those who have not received medical services from 1 January 2017 - 31 December 2017, if any service recipient wants to keep a copy of their medical history. You can contact us at Statistics and medical records of Phitsanulok Hospital, call 055-909888 ext. 1206 from now until January 31, 2023.

      If the deadline mentioned above has passed, Phitsanulok Hospital reserves the right to destroy the medical record file according to the next procedure.
